General Application Information

Basic Steps

All people interested in applying to the Program are strongly encouraged to contact the Program Admissions Advisor by emailing euhardy@surgery.wisc.edu to answer questions and to assist with the application process.

As you begin the application process for the UW–Madison Physical Therapy Program, please follow these basic steps:

  1. Ensure that you are eligible to apply by carefully reading the Eligibility Requirements and completing the Eligibility Checklist. If you meet all eligibility requirements, we welcome your application to the Program. You will be applying to both the Physical Therapy Program and to UW-Madison. Admission to UW-Madison is granted after applicants are accepted into the Physical Therapy Program. The UW-Madison application fee is $45; there is no Physical Therapy Program application fee.
  2. Follow the Instructions for Academic Factors.
  3. Follow the Instructions for Nonacademic Factors.
  4. Use the Final Checklist and Mailing Instructions to verify that your entire application is in order.

Selection Criteria

Selection of applicants for the UW-Madison Physical Therapy Program is competitive. Not all applicants that qualify will be admitted. Selection of applicants for the UW–Madison Physical Therapy Program is based upon:

1. The UW-Madison admission requirement of a minimum undergraduate GPA of 3.0 (last 60 semester credits); and

2. Physical Therapy Program selection criteria of:

    Academic Factors (follow Instructions for Academic Factors):

    Each of these factors are weighted 1/3 of the academic factors

    • Graduate Record Exam (GRE)

    • Science grade point average (minimum of 3.0) (calculated using those science courses listed as science prerequisites for the Program or the equivalent of those courses)

    • Grade point average on last 60 undergraduate credits

    Nonacademic Factors (follow Instructions for Nonacademic Factors):

    • Physical Therapy Program Application

    • Personal Statement

    • Patient Care Experience Forms

    • Letters of Recommendation

3. Evidence of uniqueness of background, foreign language fluency and diversity of life experiences are highly valued and strongly encouraged to be included within the nonacademic factor documents.
