Admission Information

This section contains official application instructions and forms for the Physical Therapy Program at the University of Wisconsin-Madison.

The UW-Madison Doctor of Physical Therapy Program offers two admission opportunities for its applicants. Applications are accepted beginning September 1, 2009 and continuing until January 22, 2010. Patient Care Experience Forms are accepted at any time. Applications will be accepted between January 23 and June 1, 2010 if class space is available. The Program begins a new class of 40 students each June.

Applications must be completed by the following dates in order to have the application considered on the corresponding meeting date:

  • November 13 , 2009 for action by the Admission Committee on November 30
  • January 22, 2010 for action by the Admission Committee on February 8
Once a DPT application has been received, you will be able to login to check your staus at this website.

Applicants will be notified of their selection status within 3 days of the meeting dates. The initial application deadlines are used to select highly qualified applicants. Some early applicants may have final action on their application deferred until the meeting on February 8. Selected applicants will be asked to make a $250 non refundable enrollment deposit when accepting their place in the class. This deposit is applied to the first semester tuition.

Policies and prerequisites outlined in this packet apply only to applicants for the class beginning in the summer of 2010. Applicants seeking admission beyond 2010 must request program information relevant for the expected year of admission.

The UW-Madison Physical Therapy Program complies with all federal and state laws and University policies including Affirmative Action and Equal Opportunity. University policy statements may be found on the Web at http://www.wisc.edu/wiscinfo/policy
