Instructions for Academic Factors

  1. Complete and submit the Physical Therapy Program Application and the Prerequisite Course Worksheet.

  2. Complete the UW–Madison Application and the UW-Madison Record of Residence. Return it to the Health Professions Programs - Physical Therapy (1140 Medical Sciences Center, 1300 University Avenue, Madison, WI 53706-1532). A nonrefundable $45 application fee must accompany the UW–Madison Application. Make checks payable to the University of Wisconsin-Madison.

  3. Request official transcripts from each college or university attended, including community colleges or technical schools. Transcripts do not need to be submitted from UW-Madison. Have each transcript sent directly to the Physical Therapy Program (5173 Medical Sciences Center, 1300 University Avenue, Madison, WI 53706-1532). Transcripts must be received in order for your application to be complete.

  4. Request that your Graduate Record Exam General Test scores (and TOEFL scores, if applicable) be sent to UW-Madison (UW-Madison code 1846). Official scores must be received in order for your application to be complete. The GRE needs to be current within the past 5 years and the TOEFL need to be current within the past 2 years. If you take the GRE or TOEFL more than once, the highest score submitted to UW-Madison will be used.
