Have you:

  1. Completed the Physical Therapy Program Application?

  2. Completed the UW–Madison Application?

  3. Completed the UW-Madison Record of Residence?

  4. Submitted payment of $45 to accompany the UW–Madison Application?

  5. Completed your Personal Statement according to instructions and included it with the Physical Therapy Program Application?

  6. Completed the Prerequisite Course Worksheet and included it with the Physical Therapy Application?

  7. Taken the Graduate Record Exam (and TOEFL and TSE exams within the past 2 years, if applicable) within the past five years and had scores submitted to the University of Wisconsin - Madison?

  8. Requested official transcripts from each college or university attended (including community colleges or technical schools) be sent to the Physical Therapy Program?

  9. Completed at least 20 hours of paid or volunteer patient care experience in each of two different settings? Have these been documented and evaluated on the Patient Care Experience Forms, and the forms submitted directly to the Physical Therapy Admissions Committee?

  10. Arranged for three letters of recommendation to be submitted directly to the Physical Therapy Admissions Committee? Provided a signed instruction/waiver form to each letter writer?

  11. Provided admission materials be received in the Physical Therapy Program Office and the UW–Madison Student Services and Admissions for School of Medicine and Public Health Health Professions Programs by November 13, 2008 or January 28, 2009 in order to insure a complete application?
