Admission InformationThis section contains official application instructions and forms for the Physical Therapy Program at the University of Wisconsin-Madison. The UW-Madison Doctor of Physical Therapy Program offers two admission opportunities for its applicants. Applications are accepted beginning September 1, 2008 and continuing until January 28, 2009. Patient Care Experience Forms are accepted at any time. Applications will be accepted between January 29 and June 1, 2009 if class space is available. The Program begins a new class of 40 students each June. Applications must be completed by the following dates in order to have the application considered on the corresponding meeting date:
Applicants will be notified of their selection status within 3 days of the meeting dates. The initial application deadlines are used to select highly qualified applicants. Some early applicants may have final action on their application deferred until the meeting on February 18. Selected applicants will be asked to make a $250 non refundable enrollment deposit when accepting their place in the class. This deposit is applied to the first semester tuition. Policies and prerequisites outlined in this packet apply only to applicants for the class beginning in the summer of 2009. Applicants seeking admission beyond 2009 must request program information relevant for the expected year of admission.
