If you are scheduling for a specific physician, please mention that when calling.

Your first appointment at the UWPTRC must be arranged by referral from your primary doctor or another provider involved in your care. Ask your doctor to call (608) 263-9550 for information and to request an appointment.

Once the appointment is scheduled, we will send you some questionnaires to fill out and bring to your first visit. This will allow us to spend more time on your pain problem and less time reviewing your medical history.
